Enable the UTF-8 BOM. Excel assumes a legacy encoding without it, which is a long-standing Excel quirk rather than a fault in the file.
They are joined into a single cell separated by semicolons. CSV is flat, so genuinely nested data has to be compromised somewhere.

That record did not have that key. Columns come from the union of all records, so gaps are expected.
